Did you ever open and modify FileVista.mdb with Microsoft Access?

No, I did not. However, When I got with my Sys admin team to check the logs of the server, it was reported that there was an I/O error. I copied the .mdb file and opened it with MS access to see if it is even readable, and it was. As a test I restored a previous version of the .mdb file (about 3 days old) and setup an eval site pointing to the same root structure and was able to login. I have sent an email to the customer explaining that there maybe a potential Database corruption and that a restore from a previous version would alleviate the login issue, user accounts created after that date of restore would have to be recreate. I Am awaiting a response.

You should try to fix the current database file by using "Compact and Repair" feature within MS Access.

Yeah, I tried that earlier on in my troubleshooting and it did not resolve the issue. So I had to settle with a restore.
