Layer between Upload files interface and upload process

Does somebody know a way to plugin a layer between the upload files interface and the upload process ?

We want to present some questions and an agreement checkbox to the uploader before an upload happens.
The result of the questions + info about the uploaded files need to be stored in a database, so it needs to be possible to write code in the layer.
